Proactive Health Care Services is a health industry that provides holistic healthcare, such as Primary Care, Behavioral and Mental Health Services, and Residential Services Agency. We take the three sub-divisional concepts and merge them into a bigger entity. We have the understanding that our population needs at least one of the services or all three. We provide one-stop services for all healthcare needs. We provide services for all ages.Our office hours are Monday through Friday, 9:00 am through 5 p.m.
